Question: my mouth got pasty this eveing and when i went to bed i notice my taste buds looked like they are peeling off where is it peelig off is shiny and there is no texture. i will admit im not a regular brusher never flossed and am a smoker. but it is causing a type of bruning sensation what could it be?

Answer: Without seeing it, I would guess one of two things. If it is only in the center of your tongue, it could be median rhomboid glossitis. If it is spotty all over your tongue, possibly with thin white lines around the spots, then it is most likely migratory glossitis which is also known as geographic t . . .

Noncontact Ultrasound Therapy for Adjunctive Treatment of Nonhealing Wounds: Retrospective Analysis
Background and Purpose

The optimal adjunctive therapy for wounds that fail to heal despite conventional wound care has not been established. Clinical evidence suggests improved healing in wounds treated with noncontact ultrasound therapy (NCUT). Although existing evidence supports the use of NCUT for enhanced wound healing, the total number of participants studied remains modest. This study was conducted to assess the impact of adjunctive NCUT on the healing of wounds that fail to progress to healing with conventional wound care alone.

Participants and Methods

A retrospective review of charts for 76 patients who had received outpatient wound care at a single center between January 2005 and December 2006 and who were treated with NCUT as an adjunct to conventional wound care was conducted. All wound care interventions used during the study period were assessed. The primary effectiveness endpoint was the percentage of change in wound area from the start of NCUT to the end of NCUT.

Results

Noncontact ultrasound was administered for a mean of 5.1 minutes per session for a mean of 2.3 times per week. The median duration of therapy was 4.3 weeks. The median wound area was reduced by 79% from the start of NCUT to the end of NCUT (from 2.5 to 0.6 cm2). The proportion of participants with greater than 75% granulation tissue increased from 32% before NCUT to 46% after NCUT.

Discussion and Conclusion

The single-arm, retrospective design did not allow for comparative assessments of the efficacies of noncontact ultrasound and other wound care interventions. The use of adjunctive NCUT appears to improve healing in wounds that fail to heal with conventional wound care alone.

Instruments are needed to measure the influence of cancer on quality of life in the expanding population of long-term cancer survivors. We conducted refinement and psychometric evaluation of the Impact of Cancer (IOC) scale by use of data from a large sample of long-term breast cancer survivors and developed an instrument, the Impact of Cancer version 2 (IOCv2), to measure quality of life outcomes.

Methods

Questionnaires including 81 potential IOC scale items, the Center for Epidemiologic Studies-Depression (CES-D) scale, and the Breast Cancer Prevention Trial (BCPT) symptom scales, as well as demographic, treatment, and medical information, were completed by 1188 disease-free breast cancer survivors 5–10 years after diagnosis. We used exploratory factor analysis to identify scales and assessed reproducibility by split-sample cross-validation. Higher-order scales were extracted and all scales were evaluated for internal consistency and construct and concurrent validity.

Results

The analysis yielded a factor structure relating IOC items to psychosocial impact domains that exhibited high factor loadings (factor–item correlations of 0.59–0.94), high internal consistency (Cronbach’s statistics of 0.76–0.89), and a total congruence of 0.98 across the split samples. The Impact of Cancer version 2 (IOCv2) scales consist of a Positive Impact Summary scale with four subscales (Altruism and Empathy, Health Awareness, Meaning of Cancer, and Positive Self-Evaluation), a Negative Impact Summary scale with four subscales (Appearance Concerns, Body Change Concerns, Life Interferences, and Worry), and subscales for Employment and Relationship Concerns. Patterns of association between IOCv2 scale scores and CES-D and BCPT scores indicated good concurrent validity. Patterns of associations between IOCv2 scale scores and demographic, medical, and treatment characteristics indicated good construct validity.

Conclusion

The IOCv2 scales provide a validated tool for measuring the impact of cancer on quality of life in long-term cancer survivors.
